Please note that submitting a patient request form does not guarantee placement with a primary care provider. Patient connections are based on the availability and capacity of individual clinicians.
If a provider is able to accept you into their practice, their office will contact you directly. We appreciate your patience as our clinicians continue to care for both existing and new patients.

If you do not currently have a primary care provider, you may also register with Health Care Connect through Health811. Health Care Connect helps Ontarians find a Family Physician or Nurse Practitioner who is accepting new patients.
